Liability Waiver

RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND ASSUMPTION OF RISK I acknowledge that participating in indoor ski and snowboard training at PTO Beaverton involves inherent risks, including but not limited to: falls, collisions, equipment malfunction, muscle strain, and other injuries. By signing this waiver, I voluntarily assume all risks associated with participation in lessons, camps, and use of the revolving ski machine. I agree to release, waive, and discharge PTO Ski & Snowboard, its owners, employees, coaches, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any minor in my care. I confirm that I am physically fit and have no medical conditions that would prevent safe participation. I agree to follow all safety instructions provided by coaches and staff. I understand that this waiver is binding and applies to all sessions attended within one year of the date signed. I have read this waiver in its entirety and agree to its terms.
